48-year-old patient suffering from chronic kidney failure gets new lease of life via Deceased Donor Dual Kidney Transplant

Kurukshetra, July 15, 2025: The Organ Transplant Department at Fortis Hospital Mohali has given a new lease of life to a patient through Deceased Donor Liver Transplant and Kidney Transplant, wherein the liver and pair of kidneys of the 76-year-old deceased donor hailing from Chandigarh, were retrieved and transplanted. This is the 10th Cadaver Organ Donation case to have been conducted at Fortis Hospital Mohali. As per government protocols, one kidney has been shared with another hospital.
The Organ Transplant Programme at Fortis Hospital Mohali demonstrates the hospital’s dedication to providing world-class medical care to those in need. The programme is supervised by transplant surgeons -- Dr Sahil Rally, Consultant, Kidney Transplant; and Dr Milind Mandwar, Associate Consultant, Liver Transplant -- who have over seven years of experience performing complex transplant surgeries.
Cadaver organ donation is a labour-intensive process and can only be conducted on a patient who has been declared brain dead. The Brain Death Committee comprising four doctors assesses the patient for a non-functioning brain, unresponsiveness, absence of brain-stem reflexes and coma. The committee conducts deliberations twice (every 6 hours) to declare the patient dead.
In this case, the 76-year-old Patient was rushed to Fortis Mohali with acute haemorrhagic stroke. He was declared brain dead and 14 days later, his family expressed the desire to donate his organs. A Brain Death Committee was subsequently constituted comprising doctors at Fortis Mohali, and from Dr BR Ambedkar State Institute of Medical Sciences (AIMS), Mohali. The committee declared the Patient brain dead and the following day; the Donor Organ Retrieval
was conducted by the Liver and Kidney Transplant Team at Fortis Mohali. While the liver and both kidneys were transplanted at Fortis Mohali, the pair of eyes has been stored at the PGIMER, Chandigarh.
The kidney recipient, a 48-year-old Patient, had been suffering from chronic kidney disease for the past five years and had been undergoing maintenance hemodialysis (machine filters waste and excess fluid from the blood when the kidneys can no longer perform the function adequately). Both the kidneys received were transplanted in a single recipient in view of the donor being an extended criteria donor having a serum creatinine of 2.5 mg/dL. In such cases, both the kidneys if given to a single recipient works well. However, in this case, a Single Iiliac Fossa Dual Kidney Transplant (UDKT) was done – procedure involves transplanting two kidneys into a single recipient iliac fossa, typically the right side.
The recipient was discharged six days after the surgery with a normal graft function with a serum creatinine of 0.9 mg/dL post transplant.
The Organ Transplant Team comprising Dr Milind Mandwar and Dr Sahil Rally conducted the liver transplant surgery on the recipient. The patient has recovered fully and leading a normal life today.
Highlighting the importance of organ transplant, Dr Rally, said, “Careful selection of donors and recipients is important to have successful surgery outcomes. Except for the first three months, only a few dietary restrictions are needed. Regular follow-ups are necessary. One strategy to increase the donor pool is to include Extended Criteria Donors (Elderly donors with medical complications) and perform Dual Kidney Transplants to achieve good long-term outcomes successfully.”
Dr Rally also emphasized on the need to sensitize the medical community and educate the public at large on the process of organ donation. He also stressed upon the inclusion of trauma and neurological centres to increase organ donation rates across the state from brain dead trauma patients. There is no age limit for organ donation.
